  1. Polypharmacy for sure. Please don't refer him to me!
All of his stenting makes ya wonder if he had atypical sx leading to cath and the oculostenotic reflex was activated.
But, at least they know where his bypass grafts are.

The bane of my existence this week has been trying to locate op reports for CABGs done > 15 yrs ago.

I need a magic wand for that.

  2. This looks worse than an EMI Qualification Test Report/log.

Does the patient confirm or deny it?

  5. GOOD GOD! I'd be appalled if I didn't come across a couple of these every week. Or the infamous 'bag o' meds'

  6. How many freaking dollars has this guy cost the healthcare system? I hope he has payed for it all himself.

  7. Believe it or not this guy was just dealt a bad hand and really had many problems that he couldn't help. Unfortunatley for him we can't currently fix many of his problems so he is living on borrowed time. He did, by the way, have a primary care physician and was diligent, to a fault, in trying to maintain what remained of his health. He was one of those patients of whom it is commonly written in the chart, "Mr. X presented to the Emergency Department today in his usual state of ill health..."

  8. This is why I update both parents' lists on a monthly basis. That doesn't help any when they go out of town and donot take the list along. the one fits on a business card, the other is 23 long, that is a 4x6 card

the med people like the list, i include the md that wrote therefill-not always the original writer
